CONSENT TO USE PHOTOGRAPHIC IMAGES
 Participating in the Specialized Training of Military Parents (STOMP) Workshop constitutes an agreement by the registrants to allow STOMP and its affiliates consent to photograph, (candidly or otherwise, and alone or with others) or otherwise reproduce the registrants likeness or image and to record and reproduce the registrant’s voice, use name, likeness, image, voice, and/or appearance as such may be embodied in any photos, digital images, and the like taken or made on behalf of STOMP. In doing so you agree that STOMP and its agents have complete ownership of such pictures, etc., including the entire copyright, and may use them for any purpose consistent with STOMP missions and all advertising and publicity relating thereto. This consent is voluntary and can be rescinded at any time in writing. Please select if you consent to the photographic release

RELEASE, WAIVER OF LIABILITY
I have agreed to participate in the Parent Professional Teams Workshop (the “Workshop”) offered by Specialized Training of Military Parents (“STOMP”), a part of Partnership for Action Voices for Empowerment ("PAVE"). The Workshop is offered to selected applicants. I understand the inherent risks associated with participation in this Workshop and the possible injuries that can be sustained by participants. I knowingly and voluntarily assume these risks and full responsibility for any personal injury or illness, including death, that may be sustained by me, or any loss or damage to property owned by me, as a result of or in any way arising out of, my participation in this Workshop, or in travel to and from this Workshop. I acknowledge that I am solely responsible for medical or other costs arising out of any bodily injury, illness, or property damage or loss sustained by me through my voluntary participation in this Workshop. I hereby agree that in consideration of STOMP allowing me to participate in the Workshop, I do for myself and my heirs, executors, administrators, and assigns hereby release, waive, and forever discharge STOMP and PAVE, its Board of Directors and its officers, agents and employees (hereinafter referred to as Releases) from any and all liability, claims, demands, actions, or causes of actions arising out of or related to (i) any injury or illness, including death, that may be sustained by me or which I may cause during the course of my participation in the Workshop or (ii) any loss or damage to my property, while participating in the Workshop and any activities incidental to participation in the Workshop. I hereby agree to indemnify and hold harmless the Releases against all liability for any personal injury or illness, loss or damage to property, or costs, including court costs and attorneys’ fees that may result from or arise out of my participation in this Workshop.
